Why 49ers injuries are trending

People are searching this because ESPN published a breakdown on Oct. 7 of just how banged up the San Francisco 49ers are four weeks into the 2026 season. The Niners are 4-0, but CBS Sports has them down for the most games missed to injury in the NFL at 75. OverTheCap.com has their injured reserve, PUP and non-football injury list value as the ninth-highest in annual salary across the league. That count does not even include defensive end Nick Bosa or fullback Kyle Juszczyk, who are hurt but not on a reserve list at all.

What is known is the scope: 16 players on injured reserve and two more on PUP. What is not fully settled yet is exactly when several of them come back. ESPN says most of the 22 injured players should return at some point this season, but a handful of those windows are still rough estimates rather than set dates. What is next is Sunday, when the 49ers face the Seattle Seahawks, who are 3-1, in an NFC West matchup. That game has not been played yet.

The injury list, by designation

On the physically unable to perform list: defensive lineman Mykel Williams and running back Isaac Guerendo. Designated to return from injured reserve: receiver Christian Kirk and offensive lineman Austen Pleasants. Currently on injured reserve: defensive tackle Alfred Collins, defensive lineman Andrew Farmer II, safety Darrick Forrest, defensive back Nate Hobbs, defensive lineman Mikail Kamara, linebacker Nick Martin, defensive end Sam Okuayinonu, receivers Ricky Pearsall, Demarcus Robinson and De'Zhaun Stribling, tight end Jake Tonges, offensive linemen Brett Toth and Nick Zakelj, and defensive tackle C.J. West. Hurt but not on a reserve list at all: Bosa, Juszczyk, safety Marques Sigle, cornerback Upton Stout and defensive tackle James Thompson Jr.

Who is back soon, who is not

ESPN put the panic level at 7 out of 10. The reasoning is straightforward: most of the 22 hurt players should play again this season, but three are the exceptions. Collins tore his patellar tendon. Pearsall had season-ending surgery on a right knee issue that dated back to 2025. Tonges tore his right MCL in Week 1. None of the three are expected back in 2026.

Bosa hurt his calf in a Sept. 24 practice and Juszczyk sprained his right knee this past Sunday, and both are expected back in October. Stribling had ankle surgery after getting hurt in Week 1, and West had right knee surgery in September. ESPN says neither of those two is likely back before late November or early December if their recoveries go to plan.

What happens next

The 49ers have not played Seattle yet. That game is Sunday, an NFC West matchup, with San Francisco sitting at 4-0 and the Seahawks at 3-1. Linebacker Fred Warner framed the injury run as part of the job, saying "you never want your guys going down," and pointing out that the sport is a violent one where things happen. ESPN's breakdown also looks at San Francisco's trade deadline approach as part of how the roster manages the injury list the rest of the way, though the piece does not spell out specific trade targets.
